Work Opportunity Tax Credit and Welfare to Work Procedures

Requirements

Every hiring location and every new hired applicant must follow the procedures listed below.

Part One – Applicant

On or before the first day of paid employment, the applicant should;

  1. completely fill out, sign and date  Forms 8850 & 9061
    (either the English or Spanish side).

Part Two – HR Administrator

   1. After the applicant fills out the two forms listed above, the HR administrator        should check for;

  1. Legibility, especially SS#
  2. Completeness of forms, especially original signatures on 8850 & 9061
  3. Employee has included their physical address, NO P.O. BOXES, and
  4. If the applicant is a veteran (question 5 of the Form A), then obtain a copy of their DD-214.

Manager should then complete the “For Employers Use Only” section on the back of the Form 8850, then sign and date the form.

    2. Attach a copy of the employee’s Social Security Card AND one of the         following;

  1. Driver’s License
  2. Resident Alien Card
  3. Birth Certificate or
  4. State ID card/

    3.Completed packets should include; Forms 9061 & 8850 (ORIGINAL        SIGNATURE IS REQUIRED BY BLYMAS), copy of Social Security Card, One        other piece of Identification (see list above), and Copy of DD-214, if          applicable.

Gather completed packets and mail to Blymas!!!
