Manager should then complete the “For Employers Use Only” section on the back of the Form 8850, then sign and date the form. 2. Attach a copy of the employee’s Social Security Card AND one of the following;
3.Completed packets should include; Forms 9061 & 8850 (ORIGINAL SIGNATURE IS REQUIRED BY BLYMAS), copy of Social Security Card, One other piece of Identification (see list above), and Copy of DD-214, if applicable. Gather completed packets and mail to Blymas!!!
